The child's birth certificate, immunization records proof of residency (lease or real estate document, power bill, etc.) and the North Carolina Kindergarten Health Assessment Report form are required to register. The Health Assessment form is available from the school, the County Health Department, and most pediatricians' offices. North Carolina law requires all students entering Kindergarten receive a health assessment and all necessary immunizations.
Rising Kindergarten students who have requested a transfer should attend the Kindergarten Orientation Session in their home school district - not the school where they have requested a transfer.

Kindergarten orientation will be held in all elementary schools on Friday, March 19. Sessions start about 9 a.m. at all schools with registration beginning about 8:15 a.m. Parents will attend information sessions while their children visit with the kindergarten teachers.

Parents should contact schools as soon as possible if they have not already registered their child and make plans to attend the orientation session. Parents are to attend orientation at the school in their attendance zone – even if they plan to request a transfer to another school for next year.

Children who will be 5 years old on or before Aug. 31, 2010 are eligible to enroll in kindergarten in August.

Parents should bring these items to orientation or when they register their child:

  • An official birth certificate – not a hospital record – that may be obtained from the Register of Deeds office in the county where the child was born.
  • Proof of residency such as a driver’s license or a copy of a current lease or real estate document, power, telephone or water bill etc.
  • Every child in this State entering kindergarten in the public schools shall
    receive a health assessment. The health assessment shall be made no more than 12
    months prior to the date of school entry. No child shall attend kindergarten unless a
    health assessment transmittal form, developed pursuant to G.S. 130A-441, indicating that
    the child has received the health assessment required by this section, is presented to the
    school principal. The medical provider, or the parent, guardian, or person in loco parentis,
    must present a completed health assessment transmittal form to the principal of the
    school on or before the child's first day of attendance. If a health assessment transmittal
    form is not presented on or before the first day, the principal shall present a notice of
    deficiency to the parent, guardian, or responsible person. The parent, guardian, or
    responsible person shall have 30 calendar days from the first day of attendance to present
    the required health assessment transmittal form for the child. Upon termination of 30
    calendar days, the principal shall not permit the child to attend the school until the
    required health assessment transmittal form has been presented.
  • An updated  immunization record that provides medical evidence of the following vaccinations and the required number of doses:
    • Diphtheria-tetanus-pertussis (5 doses)
    • Polio (4 doses)
    • Measles (2 doses)
    • Mumps  (2 doses)
    • Rubella (1 dose)
    • Hib-Haemophilus Influenzae type B  (3-4 doses)
    • Hep B - Hepatitis B  (3 doses)
    • Varicella (chickenpox) (1 dose)
