2015 Community Health Assessment

Please help identify issues or factors affecting the health of Cleveland County residents. The Cleveland County Health Department is currently conducting their 2015 Community Health Assessment and seek your opinions and ideas about current and emerging issues in our community.

Please take a few minutes and go to the link below to complete a survey:

It may take 15-20 minutes to complete the survey, but if you do, you have the option to place your name in a drawing for gas cards provided by the Alliance for Health and free spay-neuter vouchers provided by Cleveland County Animal Control. Be assured that all of your responses to the survey are anonymous and confidential. Should you wish to enter your name for the incentives, that information goes into a separate file and cannot in any way be associated with your response.

Paper copies of the survey, with instructions for completion, may be found at various sites in the county: the Cleveland County Health Department, all Cleveland County YMCA locations, all Cleveland County public libraries, senior centers in Shelby and Kings Mountain, the Cleveland County Department of Social Services, the county manager’s office, and hospital front desks in Shelby and Kings Mountain.
