All requests for special nutritional needs must be submitted annually to Cleveland County Schools Child Nutrition Services on a CCS Diet Order form with a physician or registered dietitian signature.

Forms must be submitted at the beginning of each school year and/or whenever there is a diet change.

A CCS Diet Order form is available here or from your Cafeteria Manager.  Once your child’s doctor completes and signs the Diet Order, please return it to the Cafeteria Manager or to the Child Nutrition Office.  The form may be faxed to (704) 476-8308 or mailed to Child Nutrition Services at 105 East Ridge Street, Kings Mountain, NC  28086.  Upon receipt of each completed form, our Registered Dietitian will work with you and the Cafeteria Manager to plan your child’s meals.  If you have questions, please contact the Child Nutrition Supervisor at (704) 476-8128.

Allergens
Cleveland County School cafeterias do not serve peanuts or peanut products.  It is not necessary to submit a CCS Diet Order form for a child with a peanut allergy.  Orange juice is available for meals for lactose intolerant students therefore a diet order does not have to be submitted. If this information is sufficient to manage a child's food allergies, then a CCS Diet Order form is not necessary.

Carbohydrate Counts
For diabetic students, carbohydrate counts are available by contacting Child Nutrition Services at 704-476-8128.
